Definition of a form
Objective
Indicate the form's various properties and define the data sections that make it up.
Data
The form has the following properties:
Sequence number of form Number used to order the forms defined by the school. It is required.
Code of form Unique code identifying the form. It is required.
Name of form Descriptive name of the form. It is required.
Print heading Option to print the header of the form. This header includes form name displayed in large characters inside a frame.
Category It is possible to associate a category to the form, to group the forms together by subject. The choices offered in the pulldown menu are the various categories defined in the Form categories activity, in order of category code. ("None" by default).
Completed by Types of personnel authorized to complete the form in the student record. The selection is done with the icon which calls up the Selection of types of personnel screen.
Viewed by Types of personnel authorized to view the form in the student record. The selection is done with the icon which calls up the Selection of types of personnel screen.

The personnel restricted to viewing the form will not be able to modify the data in the student's form (except in sections where it is indicated that they are authorized in modification) and they will never view the confidential data and notes that have been entered, if such is the case, by the author.
Copies management This option indicates if one or more copies of the form can be completed for a given student during the year and it determines the type of security covering the authors (authorized personnel completing the form):
- One individual copy
Each author is limited to one copy of the form per student. Only the author is authorized to modify their copy.
- Several individual copies
Each author can complete several copies of the form for each student. Only the author is authorized to modify their copies. 
- One shared copy
Only one copy of the form can be completed for each student. If several authors are authorized, they will share the same copy.
- Several shared copies
Several copies of the form can be completed for each student. If several authors are authorized, they will share all copies.
The personnel authorized to view have access, to view only, to all existing copies of the student's form, regardless of the author.
Context-sensitive help Context-sensitive help relating to the whole form. The icon displays the Extended editing screen used to complete the text in a larger area. It will be possible for the form's users to consult the text by using the icon.
For each section making up the form, there is the following information:
Sequence number of section Number used to order the sections within the form. It is required.
Name of section Name describing the section.
Repetitive section Indicates if the section is simple or repetitive.
Actions
Add section The Add command displays the Section screen used to define a new section in the form.
Access section The command displays the Section screen used to modify or view the properties as well as the structure of the form.
Save The Save command records the changes entered on screen in the database.
Delete sections The Delete commands eliminates from the database the sections selected on screen. A confirmation of the deletion is required.

None of the data in the section to be deleted can be completed in student forms. Deleting a section automatically entails deleting the data in the section.
Undo The Undo command cancels the changes entered in the page.
Agenda The Agenda command displays the Agenda screen used to request that the system automatically generate, in the selected personnel's agenda, at the dates indicated, an event concerning the form and the students for whom the form will be completed or modified.

When the copy-management mode for the form is One individual copy or Several individual copies, the Agenda screen is accessible only if the form contains at least one data of type Date (in which case at least the "Form author" selection will be offered to designate an authorized personnel).

When the copy-management mode for the form is One shared copy or Several shared copies, the Agenda screen is accessible only if the form contains at least one data of type Selection list (Personnel) and at least one data of type Date (in which case the "Form author" selection will not be offered to designate an authorized personnel).
